CSA MEAT CLUB SUBSCRIPTION FORM

Please fill out this form completely, including payment information. By submitting you are agreeing to the terms and conditions of the Marin Sun Farms CSA Meat Club.

Members must pick up packages within the time period alloted for their specific pick up site.
Packages not picked up cannot be returned and members shall not be entitled to a refund for same.
MSF is not responsible for any packages after they are delivered to the pick up site.
Subscription payments are final upon receipt and are non refundable for any reason.
Members may choose to down grade their package at any time, but no refund or credit will be given.
Members' credit cards will be automatically charged for all additional items ordered. A receipt will be included with members monthly package.
